Are you setting the created from field to the ID of the PO that gets created?

Could you provide more information? What is CO-MAN? Another company's NS? What error are you getting? What other steps are in your flow? For your question, can you provide more info on how you think a lookup might help here? For example, you'd use a lookup to either refine or enrich your original data, such as pulling order info from NetSuite then calling into Salesforce to get details on the customers for those orders. To get your issue resolved more promptly, I would recommend submitting a support ticket.

Yes, Co-Man is our Contract Manufacturer that produces products for our company. We were able to send the PO from our NS to the Co-Man NS. The Co-Man created the Item Receipt and is trying to send it back to our NS to mark it received. Here is what we have setup thus far

There's no way to tell what's going on based on the information you've provided. You might want to take the integrator IO courses. Creating fulfillments is one of the trickier integrations since it's a record transformation in NetSuite.
